The Social Workers/Case Manager acts as a liaison and resident advocate for resident and family members regarding individual issues in conjunction with other departments to ensure that your personal, social, and emotional needs are being met. We continually monitor for changes in mood, behavior, and cognition. The Social Workers/Case Manager will work with the Care Plan Team to coordinate individualized treatment Care Plans for residents in the facility. Social Services Staff will meet with residents and family members to discuss intervention to address any needs.
